Registering as a Patient

 

We require photographic identity together with proof of your address, provided you live within our practice area, our receptionist will register you with the practice. We would ask that where possible you avoid our busiest period, ie: 08.30 - 10.00, when coming to register as a patient.

You may express a preference to receive medical services from any of our doctors or practice nurses; however, this will be dependent upon availability or appointment and the urgency of the problem.

We operate across two sites, you may be seen in either surgery. Both sites have full access to all our patient records.

To register at the practice please fill in the GMS1 form, link below and provide 1 form photographic ID and 1 form of proof of address.

Please also fill in the new patient questionnaire form which you can obtain from reception until it becomes available online.  Thank you
